Qualification Form

Assessment products listed below that are distributed by ADD WareHouse are restricted to B-level qualified purchasers in accordance with the ethical and professional standards of the American Psychological Association and the Standards for Educational and Psychological Testing. Purchasers of B-level assessment products must have completed graduate level courses in tests/measurement or have received equivalent documented training.

Purchasers of these assessment products must complete the Online Qualification Form below and send their completed form request to ADD WareHouse to establish a qualification level. Your form will be reviewed and qualified orders will be processed immediately.

Explanation of Qualification Levels for Test Products

Qualification Level: B
Purchasers of B-level tests must have completed graduate level courses in tests/measurement or have received equivalent documented training.

Qualification Level: C
Purchasers of C-level tests must meet B-level qualifications and must have training and/or experience in the use of tests and have completed an advanced degree in an appropriate profession (e.g., psychology, psychiatry). Additionally, depending on state requirements, membership in a relevant professional organization (e.g., APA), or a state license/certificate in psychology or psychiatry may be necessary. Test-specific qualification criteria may also apply.
